The Benefits Website. This is where you will change your insurance benefits when you start, as a result of qualifying events, and during Open Enrollment. You can use the “shopping cart” feature to see how choices will affect your payroll deductions here as well.

You’ll need to update your address in two places:
Go to My Profile, then click Edit next to the Personal section. Make the change, and click Save.
Click Profile, select Addresses from the left menu, click Edit next to the address. Make the change, and click Save.

It is important to make sure no changes need to be made before approving your time. It’s more difficult to fix afterwards.
If there is an issue, first reach out to your manager and regional.
Payroll is processed on the Wednesday of a payroll week, so if any changes need to be made after Tuesday, email Karin Dunne.

Changes to enrollment including dependents can be made in the Benefits Website.
You can make changes to your enrollment when you are a new hire, during open enrollment, or if you have a qualifying event. Qualifying events include births, adoption, marriage, divorce, gaining/losing other coverage. You must make changes within 30 days of a qualifying event.

This website gives helpful information on the difference between an HSA and an FSA: HSA vs FSA
Our FSA program is through Paychex . Open Enrollment begins in the fall of each year and you manage your account through Paychex Flex. You will receive an email each year reminding you of Open Enrollment. Please reach out to Karin Dunne if you did not receive this email. All changes to the FSA will be made through Paychex Flex. For 2023, the maximum amount you can contribute to an FSA is $3,050.
To be eligible for an HSA, you must have one of the High Deductible Health Plan options. If you are unsure if you have a qualifying account, please reach out to Andy McDermott at amcdermott@laains.com . You will then need to open an HSA bank account. We have an option through Bank of America (email Karin Dunne for information) or you can open any HSA account. Then send the bank account information and the amount you want to contribute to Karin Dunne. You can change your contribution as necessary by emailing Karin Dunne. For 2023, the maximum amount you can contribute to an HSA is $3,850 for self-only and $7,750 for family. Those 55 and older can contribute an additional $1,000.

All full-time employees are covered by Banyan for short- and long-term disability. There is an elimination period of 15 days, so if you are going to be out for more than that time for a medical issue, please reach out to your manager, copying Karin Dunne, to get the form and instructions. These are filled out by you, Karin Dunne, and your physician, then submitted to Mutual of Omaha for payment.
